For the quantitative measurement of the percent of glycated hemoglobin (%HbA1c) levels using a fingerstick blood sample. This test is to be used by people with diabetes to monitor long-term glucose control. Monitoring your A1C tells you and your doctor how well you have been controlling your glucose levels over the past 2-3 months.

*Treatment guidelines recommend testing 2 times per year if your blood sugar levels are controlled, and 4 times per year if you are on insulin, your blood sugar levels are not controlled or your treatment changes.
Choice DM Test Limitations:
This test is NOT for the screening or diagnosis of diabetes
A1C Home Test can only be used once
If you have high levels of hemoglobin F, S, or C (or any other variant hemoglobin), you may get incorrect results
This test is not a substitute for regular visits to your doctor or for monitoring your glucose levels
If you have hemophilia or are on anticoagulation therapy, talk to your doctor before using this test
This kit contains all materials necessary for one A1C home test, except cotton balls or gauze and a bandage. All materials in this kit are disposable. Use one time only and then throw away. Store below 86 degrees fahrenheit.
